Primary Care
Primary care encompasses a broad range of health services, including diagnosis and management of illness, health promotion, and disease prevention. These services are provided by a multidisciplinary team including family physicians, nurse practitioners, nurses, and other members of the allied health team. This includes:
Regular check-ups and routine care: immunizations, general health assessments, pregnancy and post partum, well baby and child visits
Urgent care: treatment of common injuries and illnesses (e.g. infections)
Cancer screening (e.g. breast, colon, cervical)
Chronic disease management: providing ongoing care for chronic illnesses such as diabetes and high blood pressure
Mental health: providing support for mental health concerns
Referral to specialists and services when needed
Mental Health
At the Rideau Family Health Team, we recognize that mental health is just as important as physical health. Our dedicated mental health professionals— including social workers and psychotherapists— offer brief 12-session individual therapy using evidence-based approaches. Some of the common reasons patients are referred might be:

Relational concerns
Grief and loss
Life stressors and challenges
Together, we’ll support your journey to better mental wellness in a safe, respectful, and confidential environment. Patients may also be connected to other relevant services in the community.
Resource Navigation
It can be difficult for people to know what support is available to them in the community.
Our resource manager is the key provider for navigation support and coordination of health care resources at the Rideau Family Health Team, in collaboration with our primary care physicians and the team. This includes helping individuals and families navigate and connect with appropriate providers, support services, community-based organizations, and other sectors of the health care system including public health, mental health, addictions, long-term care, hospice care, respite care, social services, children’s services, home & community care, and developmental services.
The resource manager will offer direct support to patients and families and help with:
Navigation of local services and supports
Identifying your needs and any barriers
Helping you to find and access needed resources and support in a timely manner
Understanding eligibility and criteria for available community programs and services
Finding qualified providers in the community
Nutrition and Diet
Healthy eating is fundamental to our health and an important tool for reducing the risk of many chronic diseases. Navigating this topic can feel confusing at times, especially with the amount of nutrition advice and information that surrounds us all today. When working with our Registered Dietitian you will receive personal advice about food and lifestyle choices, and how these can influence your health. Our Dietitian will help you identify eating habits and patterns, provide clarity on food and nutrient needs, and guide you to develop the knowledge and skills required to make the most out of your food choices.

Living with and/or preventing diabetes
Supporting parents of picky and selective eaters
Heart health, including blood pressure and cholesterol management
Malnutrition and increasing weight when appetite is poor

Our Women’s Health Program is nurse practitioner led and designed to support a range of women’s health needs.

Managing Medications
Medications are an important part of managing your health but can become overwhelming – whether by the sheer number of medications you may be taking or by the complexity of your regimen. Our pharmacist-led Medication Management program will:
Provide a thorough, accurate list of your medications (including non-prescription products) with information gathered from speaking to you directly, speaking to your pharmacy, and looking at your chart history - including specialist notes.
Ensure you are taking your medications for the right reason.
Ensure doses are appropriate and that there are no problematic drug interactions.
Help monitor effectiveness or side effects of certain medications (e.g. arranging for bloodwork).
Help resolve any drug-related programs you may be experiencing.
Smoking Cessation
Quitting smoking/vaping can significantly improve your quality of life; we understand this is a huge decision and provide one-on-one, judgement-free counseling to help with your Quit Journey.
This pharmacist-led program will help create a quit plan that meets your needs. Even if you’re not ready to quit, our program can support tailored smoking/vaping reduction goals by providing counseling on habitual/behavioural concepts and closely following up with you.
Additionally, medications can significantly increase your chances of Quit and remaining Quit. We can prescribe smoking/vaping cessation medications such as Champix, Nicotine Replacement Therapy (NRT), or Zyban.
The Rideau Family Health Team is part of the STOP program; this allows you access to FREE Nicotine Replacement Therapy (NRT) products (Nicorette/Thrive Patch, gum, lozenge, inhaler).
Diabetes Care
The Rideau Family Health Team’s Diabetes Team includes our Registered Dietitian and Pharmacist. Whether you have questions about your medications, monitoring your blood sugar and/or planning your nutrition, we are available to support you in optimizing your diabetes care by:
Providing straightforward, practical information on nutrition that is tailored to your lifestyle.
Ensuring you are on the best possible medication regimen for your diabetes.
Helping adjust your insulin dose appropriately.
Teaching how to independently use and interpret devices such as glucometers, continuous glucose monitors (Freestyle Libre, Dexcom) and injectable medications.
Providing follow-up when needed and communicating concerns with your physician.
